  • Population-health oriented policies

    population-health oriented policies

Submit Section III of the case study, in which you will address the chosen organization’s financial incentives and quality improvement processes. Specifically, you will critique the organization’s population-health oriented policies, the organization’s approach to care and the extent to which it is patient-centered, and any strategies the organization uses in its population-health approach.

USE THIS for the chosen organization>>>>healthcare organization presented in the Integrated Safety-Net Health Care System case study using a population health approach. Please also read the supplemental Interpreting Services Program document to ensure that you have all of the information about the Montefiore Medical Center you will need to address the critical elements.

Specifically the following critical elements must be addressed in population-health oriented policies paper:

III. Financial incentives and quality improvement processes

    a) Analyze population health-oriented policies (such as non-discrimination in hiring, care and treatment, Patient Bill of Rights, financial assistance, etc.) that the organization may have implemented for reducing costs and improving overall quality. Be sure to provide evidence to support your claims.

    b) To what extent is the organization’s approach to care considered patient-centered? Be sure to cite specific examples to substantiate your claims.

    c) What specific strategies (such as eliminating unnecessary procedures and providing only essential treatments or interventions) does the organization employ in its population health approach to reduce costs and improve overall quality?

    d) Describe gaps or deficiencies that may exist in the organization’s use of the population health approach in reducing costs and improving quality of care, and provide evidence to support your claims. If you feel there are none, be sure to explain your reasoning.

Guidelines for Submission: Milestone Three must be submitted as a two- to three-page Microsoft Word population-health oriented policies document with double spacing, 12-point Times NewRoman font, one-inch margins, with sources cited in APA format.

  • Pediatric Cardiac Surgery and PICOT Question

    Comment on the following Pediatric Cardiac Surgery and PICOT Question;

When reviewing the case study there are a many key points that are discussed in regards recovering from a pediatric cardiac surgery and the risk for a cardiac tamponade after the removal of a pulmonary artery catheter. There are a few pieces of information in this case study that are critical in determining how one would find the evidence, that is most appropriate, to determine how a complication, such as a cardiac tamponade, could occur in an infant only four days after an operative procedure. The key clues include: an infant who is four days postoperative after a cardiac surgery, abnormal clotting profile, removal of a pulmonary artery catheter with drainage and ooze, followed by a deterioration in the patient’s status, which ultimately leads to a diagnosis

of a cardiac tamponade (Melnyk&Fineout-Overholt, 2015). To enhance the likelihood of the most reliable evidence and helpful research, one would use a PICOT formation, to formulate a research question. The P in PICOT stands for the population at interest (Melnyk&Fineout-Overholt, 2015). In this case study, the population the study is aiming toward is an infant four days postoperative (Melnyk&Fineout-Overholt, 2015). The next searchable topic would be the I, C, and O, which would indicate the issue at hand, the comparison of that issue, and the affect this event would have on the outcome; this would include questioning the frequency of removing a pulmonary artery catheter, or not removing the catheter, and the affects this procedure has on influencing a cardiac tamponade (Melnyk&Fineout-Overholt, 2015). The next, and final step would be to determine the T, which delivers a timeline for an event to occur (Melnyk&Fineout-Overholt, 2015). In this case study, the timeline would be four days after surgery, which one would possibly be able to broaden to a week, to better serve research results. In a study conducted in 2012, the authors described the importance of formulating a comprehensive PICOT when researching a topic at hand (Riva et al.). This study discovered that the more detailed and specific a PICOT was, the more evidence and research was exposed to better enhance the topic of understanding (Riva et al., 2012). This study also concluded, the more likely the researchers used the PICOT formation, the more likely one would develop a more appropriate and specific research yield (Riva et al., 2012). With all this information, for a Pediatric Cardiac Surgery and PICOT Question, the most suitable PICOT formulated question, to determine the likelihood of an event similar to this would be: In an infant who recently had cardiac surgery, what is the frequency of cases regarding the removal of a pulmonary artery catheter, versus not removing the catheter within a week, have on the affect of developing a cardiac tamponade within the first week of surgery? This formation of questions and information would best suit the research topic at hand. Pediatric Cardiac Surgery and PICOT Question

  • American Cancer Society Information

    American Cancer Society Information

The American Cancer Society can act as a resource to newly diagnosed cancer patients by providing unbiased information. Information is presented in non-medical terms and is easily accessible. Their website has information from prevention to treatment and all steps in between including side effects from treatment. The ACS website offers information on financial assistance as well as resources for family members who may be affected by the new diagnosis. The link for getting involved provides people with options to donate money or their time. Volunteering with an organization such as the ACS can be quite beneficial for those affected by cancer as well as connecting people who are facing the same diagnosis.

Factors that include incidence and mortality rate include poor diet, sedentary lifestyle, being overweight or obese, use of tobacco and/or alcohol, living a high-risk lifestyle, coexisting infections such as HIV and Hep C, and sun exposure. Changes that we are seeing to reduce incidence and mortality include increased education and awareness, access to screenings (colorectal and/or cervical), vaccines (HPV), and treatment of those coexisting infections.

The research topic that I found was related to the financial costs of smoking-related illnesses and their impact on society. It’s estimated that society faces a loss of $422 billion in health-related expenses. The importance of this particular study was the focus on low and middle-income countries, the first of its kind. These countries account for 40% of the cost mentioned. By bringing this to attention hopefully we can provide more education and resources to these areas.

  • American Cancer Society funds Clinical Cancer Research

American Cancer Society funds Clinical Cancer Research

Imagine that a family friend or colleague has just been diagnosed with cancer. Explain how the American Cancer Society might provide education and support. What ACS services would you recommend and why?

The American Cancer Society helps patients, families and anyone that is involved in the care of a loved one or themselves while dealing with cancer. The American Cancer Society help people to understand what to expect and what might help to cope with this devastating diagnosis. They provide basic information about any cancer and its causes as well as detailed information on specific cancer types. The American Cancer Society also provide information about risk factors, early detection, diagnosis, and cancer treatment options. On their website, they provide information on how to stay healthy, treatment and support as mentioned before as well as how to get involved and their research.

A service from the American Cancer society that I would recommend is “Tools to Help you Navigate Treatment.” Being in shock and going through the grief stages from finding out someone has cancer is tough. Adding that to treatment options like choosing a treatment team, and insurance resources may make it more overwhelming for the patient and their loved ones. On their website, they help you understand the healthcare system. They have links where you can read about what questions to ask your healthcare provider, they explain the doctor-patient relationship and they also explain about healthcare professionals associated with cancer care. Under that same page, they will have information about directories of healthcare professionals, treatment centers, and how to choose a doctor and a hospital that fits your needs. For parents who have a young child with cancer, the American Cancer Society also has information about pediatric cancer centers that will be more appropriate for their care and age. One of the other things that they have under support is that they provide information about choosing home care agencies instead of hospital treatment. They also provide information about hospice care, which is one of the last resources when a patient is suffering from cancer.

According to statistics published by the American Cancer Society, there will be an estimated 1.5 million new cancer cases diagnosed each year over the next decade. What factors contribute to the yearly incidence and mortality rates of various cancers in Americans? What changes in policy and practice are most likely to affect these figures over time

Factors that contribute to yearly incidence and mortality rates in Americans are: diet, exercise, stress, tobacco, infectious agents, and environmental factors. For example, men who smoke are more than 20 times likely to develop lung cancer than men who don’t smoke (American Cancer Society, 2017). It is important to keep in mind that many other factors like age, gender, family history and lifestyle choices may increase the risk of someone getting cancer. Changes in policies like offering healthier meals at schools may help in lowering cancer statistics. Teaching children about healthy habits like eating healthy foods, and exercise might prevent them from smoking, lowering stress levels, and even if they are at risk, they will lower their chances of getting cancer.

Select a research program from among those funded by the American Cancer Society. Describe the program and discuss what impact the research will have on the prevention or treatment of cancer.

The American Cancer Society funds the Clinical Cancer Research, Nutrition, and Immunology. In this program, scientists investigate new ways to identify and monitor cancer, explore how the immune system can be used to fight cancer, and increase our understanding of the effects of the environment on cancer, among other areas of research (American Cancer Society, 2017).
