Evidence Based Practice in Disaster Planning Essay

Public health surveillance is one way that public health officials target intervention strategies (Turlock, 2016). Often, it is through prompt recognition of and reporting of incidents of communicable disease that a disaster can be averted (Turlock, 2016). Surveillance activities often prompt questions such as, What is causing the disease? How is it spreading? And who is at risk (Turlock, 2016)? While it is true that preparedness planning cannot eliminate all traces of threat to a community, planning assures that medical services and treatment are deployed in an effective, efficient, and rapid manner (Turlock, 2016). Public health plays a vital role in coordination of providers, assurance of supplies particularly when the Strategic National Stockpile pharmaceuticals and supplies are required, and mobilization of state and national response systems. Public health officials may also provide health care services when required (Turlock, 2016).

Stanhope (2016) noted that evidence-based practice (EBP) has become more important in health care for many reasons: increased expectations of consumers, increased availability of information through the Internet, increased accountability for results, health care economic changes, and growing numbers of lawsuits, among other reasons. EBP is a lifelong problem-solving approach that regularly produces excellent results and often provides the theoretical underpinnings for programs to mitigate problems in the community. Once programs are in place, evaluation of their effectiveness should be conducted to determine whether they are worth the continued expenditure of resources. Use of EBP is vital to assure safe outcomes for populations during disasters, such as massive communicable disease outbreaks, and should be the foundation of disaster-planning strategies.

Review websites where a disaster plan may be available for the public, or if one is not currently available, call public health department to see if a disaster plan exists for your community and what the plan contains.

In addition to reviewing websites for information about your local disaster plan, you will need to locate best practice/evidence-based practice guidelines in professional literature to determine whether your community’s disaster plan is as sound as it might be or if there is room for improvement.

Section 3: Is My Community Prepared for a Disaster?

What conclusions can you draw about your community’s preparedness plan from having completed this evaluation?

  • Implementing Six Sigma At Wishmewell Hospital Assignment

Assignment 1: LASA 2: Implementing Six Sigma at Wishmewell Hospital

Emergency departments (ED) at many hospitals have been overwhelmed in the past year, as more patients without health insurance use the ED as a primary care solution. Wait times in the ED are increasing as more and more Americans are using the Emergency Department for their general health concerns. Across the country, the average ED wait time is now 222 minutes (approximately 3 hours, 42 minutes).

Wishmewell Hospital’s average wait time in the ED is more than five hours, and the board of directors is concerned about this long wait time. The danger is that a patient’s condition may escalate during his/her waiting time in Wishmewell’s ED.

The board has tasked you to take on a new quality improvement initiative to decrease wait time in the ED using the Six Sigma approach.

Develop a quality improvement program using the Six Sigma approach to decrease waiting time in Wishmewell’s emergency department.

The quality improvement program must include the following elements in an approximately 10-page Implementing Six Sigma At Wishmewell Hospital Assignment:

Describe the goals and objectives of your plan to decrease ER wait times at Wishmewell.

Describe each step of the Six Sigma (DMAIC) process:

    Define the problem

Measure the process; what Six Sigma tools will you use?

Analyze the data; what tools will you use to analyze the data?

Improve the process; explain your improvement plan

Control; how will you continue to monitor the results and adjust as necessary?

Describe the key players (stakeholders) who should be members of your implementation team and explain why teamwork is an important factor in implementing a quality improvement program.

Explain at least three factors that might inhibit the implementation of your decreased wait time program.

There are a lot of similarities and key differences between the Nurse Practitioner (NP) role and that of Physician Assistant (PA).  Both NPs and PAs are able to diagnose, educate, treat disease, prescribe and typically work in similar settings in the outpatient private practice.  However, PAs always prescribe and practice under the supervision of a physician.  NPs on the other hand work in collaboration with physicians in some states and in others they are able to work completely independently and own their own practice.  The Advanced Practice Nurse has many specialty routes such as CNS, which provides care to patients with complex diseases/conditions, improves patient care by developing clinical skills and retains nurses that are subject matter experts (Hamric, Hanson, Tracy & O’Grady, 2014).  Advanced Practice nurses can also specialize in acute care, nurse-midwife or nurse anesthetist.  Both PAs and NPs need to be certified, and PAs become nationally certified through taking the Physician Assistant National Certifying Exam (PANCE), obtain 2000 clinical hours and they must complete a re certification exam every 10 years (ASPA.org, 2017).  Nurse Practitioners must have a minimum of RN license, MSN or Doctorate degree and become certified through taking tests with either American Nursing Credentialing Center or American Association of Nurse Practitioners.  PAs on the other hand can hold a Bachelors degree, however often times they hold a Master’s degree.

Health gap between is the biggest controversial problem in country’s health sector. The number of premature mortality rate is seemingly different from one state to another due to the unbalanced kind of health opportunities.

    California Health State Problem

California State population is considered to be approximately 38 million, but an alarming factor is the high number of people who have diabetes, adult obesity, adult smoking and hypertension rate. The age of rate of California is considerably an aging population where it is estimated that about 25% of the population in California will be above 65 years old in the year 2060. This will also increasingly increase the number of chronic diseases to the extent of country crises(Mudrick, Breslin, Liang & Yee, 2012).

The health indicators in California show that; adult obesity has increased with 3.1%, diabetes rate has also increased with3.3 %, and hypertension has also increased with 4.5% rate. The State health has increasingly got worse with reporting of new cases of blood pressure, obese and diabetic reported in hospitals daily. The rate of growth of this chronic diseases and the aging factor will cause a major a health problem that can cause a national medical crisis (Mudrick et al., 2012).

What are health resources currently available to address the problem?

The reforms carried out in the State are the insurance designs, wellness incentive programs, provider organization structure, and provider payment method. The programs are initiated to curb the financial crisis of the problem and also to boost the healthcare programs in curbing the gap in the California state (Williams, Goodwin, Baillargeon, Ahalt & Walter, 2012).

What health policy might you suggest to improve the ranking in that area?

The transparency of California health sector should amend by implementing “all-payer claims database.” The State will be able to monitor private and public health programs and plan in the state.  The policy will increase the efficiency of healthcare through the broader information spread into the healthcare market thus increasing efficiency through improving payment models.

  • Dade County Health

Respond to the following two peer articles by student on Dade County health

NB: These are written by students, not our writers.

    1.

Florida has always been a mekka for vacation and leisure activity. It is a diverse city that has a large multicultural and very fast pace living. According to the Florida Health ranking and roadmap, Florida’s STD rankings are getting worse every year. According to the ranking Florida’s rank hast skyrocketed from 2007 being 214% in Miami-Dade alone and is now skyrocketed to 410% in 2014. This statistic is very alarming and indicated our sexual reproduction system needs attention if we want to decrease the amount of people with STDs (County Health Rankings, 2014)

In Miami Dade county the amount of uninsured has improved and has decreased from 31% in 2007 to 26 in 2014. This indicated that more of the population has access to primary care. As providers we are responsible for educating our patients each visit. I believe that sexual health screening should be part of each visit and a topic of conversation. The statistics do indicate that there is no education and no discussion going on about sexual health and practices. The promotion of safe sex ie: use of condoms and birth control should be a topic and sexual health screening should be done annually for those at risk and sexually active. The health policy that should be enforced is the screening of STD and sexually transmittable diseases. This is crucial in the stop of spread of STDs (Mahajan, Stemple, Shapiro., King, Cunningham, 2009).

    2.

According to general statistics in Dade, county health sector stills needs a lot of improvements. Lifestyle diseases specifically disease related to physical exercise and weight is still causing a crisis in the health department. Obesity and overweight in adults according to the statistics have taken a different dimension. An indicator of one having obesity in adults concluded when one has a BMI of 30 or more. However, lifestyle diseases have an antidote if precautions of physical fitness taken into account.

Dade County has shown commitment to fighting obesity which is the leading cause of death in the county. Through funding campaigns that were spearheaded 2010 by Disease Control and Prevention has realized some improvement, this happens after the rate of overweight, and obese adults hit 67.4%, 13% high school students. The overwhelming number of those affected about 25% of the adult population did not report any signs of physical exercises over the last 30days. Conversely, around 22% of adults did not accomplish the county federal government regulations for vegetable and fruit consumption. Resources available at hand to address this challenge were inclusive of revisiting childcare programs, implementation of sporting activities, installation of bicycle racks all over the country to encourage bicycle cycling and the application of sound applications of nutrition.

One of my recommendations solving this health crisis would be the implementation of healthy eating by limiting the fast food consumption and consistent physical exercise, and it would help in cutting weight. It is worth mentioning that facilitating this process would demand to educate the public on the advantages of pursuing such activities.

Alachua County experiences similar health challenges like those experienced in other parts of Florida. In Alachua County, many individuals experience poor health outcomes which are evident from the high rates of chronic disease patterns, hospitalization rates and patterns of unhealthy behaviors compelling those who seek to improve health result to interventions at the structural, policy and community level. This includes a shift towards healthier lifestyles that include quality nutrition, daily physical activity, and social support and reduced substance abuse.

Some of the health resources in Alachua are community based clinics, hospitals and even schools which bring down healthcare to the disadvantaged and people with disabilities. There are also organizations, agencies and individuals that understand the impact of social determinants of health and seek opportunities to partner so as to improve the health of the community. The main health problems are due to the socio-economic needs which could be due to unhealthy diet or due to bad social behaviors. Thus the main health policy socioeconomic disparities in health: pathways and policies and policy options on reducing inequalities in health.

Excessive alcohol use and smoking are some of the unhealthy issues that need to be addressed. Alachua community can adopt to community based programs and policies that uphold healthy living. The agenda of the health policies should be so as to reduce costly preventable health conditions, reduce health disparities and an increase in healthy behavior like productive leisure. Population based policies can also be implemented in health care delivery system by adopting insurance coverage for all in the county.
