Distal and Proximal Factors of Health Behaviors and Their Associations with Health in Children and Adolescent
How to write an Article Summary
When writing a summary, the goal is to use your own words to synthesize what you have read.
The summary should focus only on the article's main ideas and important details that support
those ideas. In a summary, you should not give your opinion or recommendations. You can give
these when you critique an article later in the semester.
    Guidelines for summarizing an article:
    • State the main ideas.
    • Identify the most important details that support the main ideas.
    • Summarize in your own words.
    • Do not copy phrases or sentences unless they are being used as direct quotations.
    • Express the underlying meaning of the article, but do not critique or analyze.
    • The summary should be about one third the length of the original article.
    Your summary should include:
    • Introduction
o Give an overview of the article, including the title and the name of the author.
o Provide a thesis statement that states the main idea of the article.
    • Body Paragraphs
o Use the body paragraphs to explain the supporting ideas of your thesis statement.
o The number of paragraphs will depend on the length of the original article.
    ▪ One-paragraph summary – one sentence per supporting detail, providing 1-
    2 examples for each.
    ▪ Multi-paragraph summary – one paragraph per supporting detail, providing
    2-3 examples for each.
o Start each paragraph with a topic sentence.
o Use transitional words and phrases to connect ideas.
    • Concluding Paragraph
o Summarize your thesis statement and the underlying meaning of the article.

Write a letter to an official in your state or local government. (Choose the individual in the level of government that will best address your issue). The purpose of this letter is to advocate for action with regards to your chosen health care environment issue.

    Collapse All

    **Introduction**

**Note:** Each assessment in this course builds upon the work you have completed in previous assessments. Therefore, you must complete the assessments in the order in which they are presented.

As health care leaders, it is important to understand strategies for advocating and lobbying elected officials. This can lead to the development of policies or laws that can help drive improved equity and outcomes for all participants in the health care environment as well as a more sustainable financial future.

    **Background and Context**

As a master's-level health care practitioner, you may be expected to implement plans to ensure that initiatives designed to take advantage of economic opportunities for the organization are rolled out successfully and can be sustained over multiple years. Additionally, it is important to be able to envision how an initiative could be implemented in different contexts and for different purposes to ensure the investment remains a viable and positive asset to your organization or care setting.

As a master's-level practitioner, you will often be challenged to influence the health care environment in a variety of ways. This influence can occur on a micro-level (implementing change on your unit, institution, community, or local organizations) or at a macro level (implementing change via state or federal regulations and policy). One way you can influence the health care environment is by lobbying an elected official at the local, state, or national level to adopt policies or legislation that would support positive economic and health outcomes for patients, practitioners, and organizations within the health care environment.

    **Instructions**

For this assessment, you will develop a letter to an official in your state or local government (choose the individual in the level of government that will best address your issue). The purpose of this letter is to advocate for action with regards to your chosen health care environment issue. Remember, when writing the letter, you must use your personal address and telephone number unless you are exclusively representing a group or your organization.

Be sure to address each main point. Review the assessment instructions and scoring guide, including performance-level descriptions for each criterion, to ensure you understand the work you will be asked to complete and how it will be assessed. In addition, note the requirements for document format and length and for supporting evidence.

Overall, your assessment submission will be assessed on the following criteria:

    · Summarize the health care economic issue that you are addressing.

    · Keep this brief but try to include details about how the issue is currently impacting the elected officials’ constituents.

    · Explain the positive outcomes that will occur if the issue is addressed and the negative outcomes that will occur if the issue is not addressed.

    · Tailor this messaging to focus on the impacts in the communities and organizations that are relevant to the elected official you are writing to.

    · Summarize key information from scholarly sources to support the importance of addressing the issue; the issue’s overall impact on health care at the institutional, local, state, or national level; and proposed changes or actions to address the issue.

    · Cite at least five current, scholarly sources that support your argument and help provide the elected official (who may not have a detailed knowledge of the health care environment) with an understanding of the issues, changes, or actions that you are proposing to drive improved outcomes.

    · Identify the impact your issue has on health care institutions and health care providers at the local community, state, or national level.

    · Make sure to include information from both a health care and an economic perspective. An elected official may be more responsive to one perspective than another.

    · Incorporate social justice, structural racism, and inequity principles when picking resources and making recommendations for change in your assessment.

    · Explain how personal, professional, and organizational experiences have informed the resource planning and risk analysis for working toward addressing the economic issue.

    · Think about how your experiences (the experiences can be personal, professional, or from your team’s perspective or experience) affect how you have planned for the resources needed to implement your desired changes or actions. How did this impact your approach to conducting a risk analysis on the project?

    · Convey purpose, in an appropriate tone and style, incorporating supporting evidence and adhering to organizational, professional, and scholarly communication standards.

    · Remember to conclude your letter with a summary of your position on the issue and a compelling reason that the elected official should act in your favor and support your issue or initiative.

    **Additional Requirements**

    ·
**Audience:** Make sure you are addressing your letter to an actual local or state official.

    · For most states, you can use the following resource:

    ·
    [Assessment 4: Lobbying for Change](https://capella.alma.exlibrisgroup.com/leganto/public/01CAPELLA_INST/lists/2490763020006731?auth=SAML&section=2491123430006731).

    · For local officials, search your city’s government website for officials and their positions.

    ·
**Length of Submission:** 1–2 double spaced pages. Your letter needs to be succinct and persuasive.

    ·
**Number of References:** Cite at least five sources of evidence to support your identification of the gap. This could be public health data, a peer-reviewed journal article, or another scholarly source.

    ·
**APA formatting:** Resources and citations are formatted according to current APA style.

**Note:** As you revise your writing, check out the resources listed on the Writing Center's
    [Writing Support](https://lat.strategiced.com/redirect?linkid=4646) page.

    **Competencies Measured**

By successfully completing this assessment, you will demonstrate your proficiency in the following course competencies and scoring guide criteria:

    · Competency 1: Analyze the effects of financial and economic factors (such as cost-benefit, supply and demand, return on investment, and risks) in a health care system on patient care, services offered, and organizational structures and operation.

    · Summarize the health care economic issue that you are addressing.

    · Competency 2: Develop ethical and culturally equitable solutions to economic problems within a health care organization in an effort to improve the quality of care and services offered.

    · Competency 4: Develop ethical and culturally equitable economic strategies to address dynamic environmental forces and ensure the future security of an organization's resources and its ability to provide quality care.

    · Explain how personal, professional, and organizational experiences have informed the resource planning and risk analysis for working toward addressing the economic issue.

    · Competency 5: Produce clear, coherent, and professional written work, in accordance with Capella writing standards.

    · Convey purpose, in an appropriate tone and style, incorporating supporting evidence and adhering to organizational, professional, and scholarly writing standards.

Create a poster presentation to communicate an implementation plan to bridge the gap between the evidence you will research and clinical practice. You will then record audio of no more than five minutes presenting your poster.

Master's-level nurses need to be able to think critically about the evidence, outcomes data, and other relevant information they encounter throughout their daily practice. Often the evidence or information that a nurse encounters, researches, or studies is not presented in the exact context of that nurse's practice. A key skill of the master's-level nurse is to transfer evidence from the context in which it was presented and apply it to a different context in order to maximize the benefit to patients in that new context.

    Collapse All

    **Professional Context**

Critical appraisal of evidence is essential for nurses to practice evidence-based nursing, improve patient outcomes, contribute to research quality, make informed decisions, and fulfill their professional responsibilities. It is a key component of promoting excellence and continual improvement in nursing practice.

Ethical research ensures that the rights and well-being of research participants are protected. This includes obtaining informed consent, maintaining confidentiality, and minimizing any potential harm to individuals involved in the research. Ethical research contributes to the long-term impact of evidence-based practice and greater acceptance of evidence-based practices. By conducting research ethically, you will generate knowledge that is more likely to stand the test of time and continue to inform practice and policy in the future.

Master's-level nurses understand the importance of utilizing evidence-based practice in their health care setting. The challenge is bridging the gap between the evidence and clinical practice. "This is the way we've always done it," is a common response and may not indicate evidence-based practice is being utilized. Furthermore, when a practice problem is identified, what are the steps to communicate the need for change with the interprofessional team?

One way to communicate the need for quality improvements, as well as your plans for achieving specific changes, is through a presentation poster. This type of communication tool is used in workplaces as well as professional and academic conferences. Being able to convey the essence and value of a project in a compelling and succinct way is a valuable skill, and it is vital within the constraints of a single poster.

    **Scenario**

You have been asked to give a poster presentation based on your work and research on a clinical problem in your practice setting. The purpose is to lay out the evidence and a potential implementation plan to your colleagues in order to bridge the gap between the evidence and the practice and improve the quality and outcomes of care.

    **Instructions**

Identify a clinical problem in your work setting and develop an implementation plan to carry out your evidence-based practice proposal using a poster presentation. You may use the

We have consistently established in the literature that a healthy work environment for nurses positively correlates not only with improved patient outcomes but also with decreased rates of attrition.  The two characteristics of a healthy nursing unit that resonate with me the most are safety, both physical and emotional, and high levels of communication and collaboration. As our textbook states, “According to the American Nurses Association, a healthy work environment is safe, empowering, and satisfying” (Yoder-Wise & Sportsman, 2023, p. 104).  Emotional safety includes avoiding cliques within the unit and leaders with open-door policies. I worked on a step-down unit that had quite a few senior nurses who had been working in this unit for many years.  They had the potential to be a wealth of knowledge and help newer nurses learn and grow in their roles.  Our manager at the time gave all of the Charge and Resource nurse shifts to these 5 or 6 nurses and never gave anyone else an opportunity to learn the roles.  It created frustration among younger nurses who were a couple of years into their careers and interested in learning new skills and roles, and not being provided with the opportunity to do so.  Emotional safety to me is largely knowing that I can trust my leadership to hear me when I need to talk or understand something.  One of my favorite leaders I have ever worked for had an open-door policy, and if you walked in to talk, she put her phone in the desk drawer.  You had her full attention.  She created a safe space for the entire unit.  And she did this for everyone; no one was ever excluded.  We had a challenging nurse on the unit who could be very unkind to others, and this manager, while still addressing inappropriate treatment of co-workers, gave this nurse the same attention as everyone else.  It created even more respect from the staff.
This is one solid way leaders can contribute to a healthy environment-simply being available to staff.  Leaders can also contribute to a healthy environment by promoting a sense of belonging and autonomy (Yoder-Wise & Sportsman, 2023).
This ties into high levels of communication.  Leadership that is willing and available to talk can make or break a unit.   A leader seeking buy-in from staff on new policies or even just the kind of culture they are trying to create needs to be willing to spend time talking with the staff and learning about them.  Collaboration is key as well.  In his book on leadership, Lee Cockerell emphasizes the importance of remembering that everyone is important.  He tells stories of money saved at Disney by allowing employees to design new processes (Cockerell, 2008).  True collaboration comes when leadership recognizes the strengths each individual brings to the unit.
One unhealthy work environment situation I have been involved in was opening an ECMO unit as part of our CICU.  Now we all have the same manager and are a cohesive unit, but at the start, ECMO had its own manager.  About 6 months in, she moved her office to another floor and was rarely seen on the unit.  Communication began breaking down almost immediately, and it caused a lot of chaos on the unit.  This leader contributed to the unhealthy environment by not being available to the staff.

Based on my nursing experience, team collaboration is a characteristic of a healthy work environment. I feel an overwhelming sense of relieve whenever a colleague of mine comes to me and asks me "Can I do something for you?", at the end of shift when I feel like I'm drowning. One simple way to foster team collaboration is by practicing yourself. Whenever you have downtime yourself, it's always important to check on other nurses who need assistance. This breeds team collaboration and makes it a normal day-to-day characteristic of a healthy work environment. Wintinessing healthcare professionals being cruel to each to each other is a situation I have witnesses too many times. For example, witnessing "nurses eating their young". This form of bullying, intimidation , or lack of support is a real issue that undermine the profession of nursing. It's a very uncomfortable situation whenever this is witnessed, because this form of abuse towards new nurses can lead to the never ending cycle of abuse. This is definitely one of the cons of nursing when it comes to becoming a new nurse or being oriented toma new unit.
Nurse leaders have a strong voice in identifying, preventing, and mitigating factor that contribute to an unhealthy work environment. Nurse leaders play a critical role in shaping the culture of healthcare teams by modeling and reinforcing healthy personal and professional behaviors. One strategy a nurse leader can implement to promote healthy persona and professional behaviors is to create an environment where team members feel safe to speak up, ask questions, and admit mistakes without ear of humiliation or punishment. Psychological safety promotes mental well-being, reduces stress, and fosters collaboration. According to Shirey (2021), psychological safety is essential for team effectiveness and professional growth, as it empowers nurses to engage in problem-solving and innovation without fear of reprisal. Another strategy a nurse leader can use to healthy personal and professional behaviors is by promoting work-life balance through flexible scheduling. Offering flexible scheduling encourages time off which can help prevent burnout and improve job satisfaction.

**Please ensure that the Reply includes more than 200 words with scholarly articles, and the plagiarism level must remain below 20%.**
    **Patient with Lowest Osteoporosis Risk**
The correct answer is **A.** Nehineza, an overweight African American female with a family history of uterine cancer.
African American women have a number of protective aspects that hinder the occurrence of osteoporosis. The group of people generally is characterized by higher peak bone mass and increased bone density than in other ethnic groups offering natural support against bone loss. The Study of Osteoporotic Fractures (SOF) and the Baltimore Men Osteoporosis Study (MOST) indicate that in both genders, blacks have greater adjusted bone mineral density when compared with whites and an age-adjusted lower bone mineral density decline rate annually (Hochberg, 2024). Moreover, being overweight also has some mechanical advantages, with a denser body weight delivering useful loading forces to bones, thus stimulating osteoblasts and increasing bone formation. The family history of uterine cancer raises concerns about other health issues, but that is not directly related to risks of osteoporosis.
    **2. T-Score Values for Osteopenia**
Osteopenia can be specified as a bone mineral density T-scores that range between -1.0 and -2.5 standard deviations (NIAMS Science Communications and Outreach Branch, 2023). This system uses bone density in the patient and compares it to bone density in healthy young adults in peak bone mass. T-scores between -1.0 and -2.5 indicate normal bone density and lower scores than -2.5 indicate Osteoporosis (MedlinePlus .gov, 2021). The intermediate stage is osteopenia which implies a decreasing bone density, not yet being medically recognized as osteoporosis but should be monitored and possibly treated.
**3. First-Line Treatment and Management Considerations**
The first line of treatment to treat osteoporosis is bisphosphonates (Dakkak et al., 2023). Generalized options consist of alendronate, risedronate, ibandronate and zoledronic acid. The drugs alternate osteoclastic action to keep the density of bones and limit resorption.
The consideration of pre-treatment is critical to the safe initiation of treatment. Renal dysfunction should be assessed because bisphosphonates should not be used in individuals with creatinine clearance less than 30-35 mL/min. Gastrointestinal examination is important especially checks on esophageal conditions and presence of active peptic ulcer disease as well as not being able to stand after use. The laboratories assessments must contain serum calcium, phosphate, magnesium, and 25-hydroxyvitamin D. It is advisable to have dental check-up to determine possible jaw issues prior to starting therapy.
Education and recommendations about patients deal with various areas. In the case of oral bisphosphonates, when the patient takes the pills, they should be taken with empty stomach using plain water first thing in the morning so that they do not lie flat for any more than 30 to 60 minutes (Mayo Clinic, 2024).Postmenopausal women should take 1200mg of calcium each day, as well as supplement it with 800-1000 IU of vitamin D daily to ensure the health of their bones (Oregon State University, 2014). To trigger bone building, weight-bearing and resistance training must be included in daily routines.

**Please ensure that the Reply includes more than 200 words with scholarly articles, and the plagiarism level must remain below 20%.**
    **Endocrine/Osteoporosis**
    1. **Which patient is least likely to be at risk for osteoporosis?**
Out of the four women reviewed, Nehineza, an African American overweight female with a history of uterine cancer in her family, is the least likely to be at risk of developing osteoporosis. The bone mineral density in African American women is higher than that in Caucasian and Asian women, which reduces their risk of osteoporosis in the whole body. Moreover, being overweight leads to increased mechanical loading of bones, as well as increased production of estrogens by adipose tissue, which exerts a protective action on bone decline (Charde et al., 2023). Granted that a family history of specific types of cancer, such as uterine cancer, is clinically significant, it does not directly affect the phenomenon of osteoporosis. Conversely, the remaining women in the sample exhibit higher associated risk factors of early menopause (Skylar), underweight and smoking (Park-Gim-Ka), and a sedentary lifestyle (Cristina), which have been highly related to a reduction in bone density and increased chances of developing osteoporosis.
    1. **Which values would be considered osteopenia?**
Osteopenia is characterized using an assessment of bone mineral density (DEXA (dual-energy x-ray absorptiometry) scan), and it is reported as T-scores. A T-score in the range -1.0 and -2.5 indicates that the bone mass is low but not so low that it qualifies as osteoporosis and is therefore diagnostic of osteopenia. A T-score between negative one and one is normal, and a T-score up to negative two and below is categorized as osteoporosis (Xue et al., 2021). Osteopenia is essential to identify, as it brings to focus individuals who may benefit from lifestyle changes and early interventions to prevent the progression of osteopenia into osteoporosis and fractures.
    1. **What is the first-line treatment for osteoporosis, and what considerations/recommendations apply?**
Osteoporosis can be treated by administering bisphosphonates, such as alendronate, risedronate, or zoledronic acid, as a first-line treatment. Lessening the danger of cracks by augmenting the thickness of the mineral in the bones is the way these drugs accomplish their task by inhibiting bone resorption and increasing bone thickness. Clinicians are expected to evaluate renal failure before initiating bisphosphonate therapy since the drugs cannot be used in patients with severely impaired renal function, such as GFR <30 mL/min. Patients should also have the ability to sit or stand for at least 30 minutes after taking oral bisphosphonates, as failure to do so can result in esophageal irritation.
The process of educating patients should involve instructing them to take the medication with a full glass of water on an empty stomach upon waking, to wait at least 30 minutes before eating or taking any other medicines, and to consume sufficient amounts of calcium and vitamin D (Foessl et al., 2023). Moreover, patients are advised to incorporate routine movements and weight-bearing activities into their lifestyle, and to quit smoking and alcohol intake to achieve the best results in terms of bone health.
