**Case Presentation: Assessment & Diagnosis (CP-AD) Assignment Instructions**

    **Overview**

This is the first part of your
**Case Presentation** **Assignment**. This is called a formative assignment in that you will get to “practice” the process of assessment and diagnosis. This is an essential first step toward the development of a sound case conceptualization and treatment plan. It is the foundation of the “golden thread” when working with a client. If you miss the mark here, it can lead your treatment plan astray and potentially do harm.

    **Instructions**

    For this
**Case Presentation: Assessment & Diagnosis (CP-AD) Assignment**,
you will complete the Assessment and Diagnosis portion of the case presentation as illustrated in the
**Case Presentation: Assessment and Diagnosis Template** and
**Case Presentation: Assessment and Diagnosis Example** documents.

**At the end of this description of instructions, you will see the detailed instructions of the content that CP-AD should have.**

You will use the data you collected during the role play (CIR) when you played the role of the counselor during that initial clinical interview. The client you interviewed during the role play (
**CIR)** will be your client for the entire project of the Case Presentation. The information you gathered during the role play will be used to complete the Case Conceptualization (CC), the Treatment Plan (TP) and the Final Presentation paper
    **.**

    As you work on the
**Case Presentation: Assessment & Diagnosis (CP-AD) Assignment**, you may have to “interview” your client to gain access to missing data points. Additionally, as you work through this
**Case Presentation: Assessment & Diagnosis (CP-AD) Assignment**, you may “consult” with your clinical team via Microsoft Teams.

Below you will find step-by-step instructions to complete this
**Case Presentation: Assessment & Diagnosis (CP-AD) Assignment**.

    · This assignment is to be 5 to 8 pages in length

    · This page count excludes a cover page and reference section

    · Please utilize APA 7th edition formatting

    · Please be sure to include headings, subheadings and intext citations

    · The assignment requires at least 1 citation, e.g., DSM-5-TR (APA, 2022). Do Not use any other version of the DSM, only the DSM-5-TR.

    · Note if other sources are used they are to be scholarly articles published within the last five years unless they are considered seminal works in the field of study.

    To complete this
**Case Presentation: Assessment & Diagnosis (CP-AD) Assignment**, do the following:

    1) Review the
**Case Presentation: Assessment and Diagnosis Instructions below.**

    2) Review the
**Case Presentation: Assessment and Diagnosis Example** document.

    3) Review the
**Case Presentation: Assessment and Diagnosis Template** document. Follow the APA formatting as provided in the template. It is developed to help you stay aligned with assignment expectations based on the assignment instructions and grading rubric.

    4) Write the Identifying Information and Reason for Referral/Presenting Problem.

    5) After collecting all the clinical data, write up the “Summary of Assessment.”

    6) After writing the summary, develop your DSM-5 TR diagnosis. Most often in clinical work this will include a primary diagnosis, a comorbidity, and Z-codes. You can “consult” with your clinical team as needed in the development of the diagnosis. Please write the diagnosis in the format indicated in the DSM-5 TR and the example provided.

    7) As you begin to establish a diagnosis, consider the definition of Mental Disorder stated in the DSM-5 TR (pp. 13-14) and consider the 4 D’s of diagnosing: deviance, distress, dysfunction, and danger. Consider onset of symptoms, duration of symptoms, frequency of symptoms, severity of symptoms (mild, moderate, severe), deviance from the norm, descriptive lineaments (good insight, poor insight, absent insight), and impairment in functioning. Consider differential diagnosis, course of the disorder (partial remission, full remission), and Risk: Suicide, Homicide, and other self-harming behaviors.

    8) After you solidified your diagnoses, then develop your diagnostic impression. The impression is an integration of DSM-5 TR criteria, specifiers, severity with supporting evidence from the case study.

Note: Your assignment will be checked for originality via the Turnitin plagiarism tool. Make sure the Turnitin report is less than 30%.

    Be sure to review the
**Case Presentation: Assessment & Diagnosis (CP-AD) Grading Rubric** before beginning this
**Case Presentation: Assessment & Diagnosis (CP-AD) Assignment**.

Please see below the detailed information that needs to be included in the CP-AD. Follow the instructions, template, and example.

Does the client have any factors such as acculturation, discrimination, etc that impact the client and may be source of signs, symptoms? How would the client explain the problem from their cultural lens?

    **Barriers to Treatment/Success**

Are there personality factors, stages of change influences, or contextual factors that would influence the success of treatment?

    **Other pertinent data**

Provide any other data points not captured from the sections above such as signs, symptoms, severity, onset, conditions, context that provide a clearer picture for the development and discernment of the diagnosis as well as client insight and motivation to treatment. As you begin to establish a diagnosis, consider the definition of Mental Disorder stated in the DSM-5 TR (pp.

    **Diagnostic Impressions**

    **DSM-5 Diagnosis**

Primary – F 32.1 major depressive disorder, single episode, moderate

Secondary – this may include another diagnosis such as GAD and also Z-codes.

    **Differential Diagnosis**

Be sure to include diagnoses that you are still assessing for to rule them out (For example, you may be ruling out Generalized Anxiety Disorder, but the client has not yet had anxiety for 6 months or longer, so you’re keeping it as a provisional diagnosis). Make sure the rationale is directly connected to the DSM-5 TR.

    **Diagnosis Rationale**

When writing up this section, make sure to offer each disorder criteria with case data to support the diagnosis. For each diagnosis, offer a separate paragraph in the diagnostic impression/rationale. Below are examples of incorrect and correct ways to write it up:

    · Not correct– The client has a marked fear about one or more social situations. The individual feared that he will act in a way or show anxiety symptoms that will be negatively evaluated (offered DSM criteria only).

    · Not correct – The client is depressed and noted sadness during the interview. The client isolated herself at home (problem, did not connect to DSM criteria).

    · Correct – The client has marked fear in several social situations as evidenced by her fear when presenting in class, turning in a paper, and speaking with classmates (A1). She is fearful to speak up when feeling wronged by her supervisor, avoids chatting with co-workers, and isolates herself at home when asked to attend social events (A2). Her fears are founded on the belief that she will act in ways that will be perceived negatively by instructors, classmates, and coworkers (B3). Offer criteria and case study data to support it.

Make sure to use Z codes as needed that are found in the back of the DSM-5 TR. At times, if no disorder is appropriate, a z-code may be the principal diagnosis.

Make sure to offer a paragraph of z-codes in the diagnostic impression/rationale.

The first paragraph is only for the principal diagnosis, the next paragraph is on the second disorder, and then additional paragraphs are for the other disorders. Each paragraph is to focus on only one disorder. It is like building a court defense. If your records are subpoenaed or you transfer a client to another counselor, they are not questioning your diagnosis as being incorrect, or inconclusive based on the diagnostic discussion. For the final paragraph, discuss your differential diagnosis.

This section should include a concise rationale for each diagnosis, differential diagnoses, and Z codes provided above.

    **References**

At least offer the DSM-5 TR as a reference.

    #### **Overview**
Polling has become a very common practice of assessing the public's viewpoint. It seems as if there's a poll for everything! But how valid are these polls? And how difficult is it to truly assess public opinion?
    #### **Assignment**
Pick a public policy issue that is of interest to you. You have been asked to conduct a poll on the issue.
    Review pollster sites, such as
    – [Gallup Organization Links to an external site.](http://www.gallup.com/home.aspx)
    – [Rasmussen Reports Links to an external site.](http://www.rasmussenreports.com/)
    – [Pew Research CenterLinks to an external site.](http://www.pewresearch.org/)
    – [National Opinion Research CenterLinks to an external site.](http://www.norc.org/Pages/default.aspx)
    – [Zogby InternationalLinks to an external site.](http://www.zogbyanalytics.com/)
and take a look at polls on your issue.
    1.  **Create** your own poll. As part of your assignment package, include the following:
    – Describe how you will conduct your poll (methodology).
    – Draft the questions you will use (content).
    – Identify potential sampling biases that could result from your poll and how you will try to avoid/prevent them (sampling error).
    2.  **Conduct** the poll, involving at least 10 people. For the purposes of this exercise, we will acknowledge that a random sampling is not realistic. Therefore, you may poll your classmates, family members, friends, etc.  Collect the data from your poll and analyze the results. As part of your assignment package, summarize your findings on the following points:
    – Summarize your results.
    – Compare your results with results from the polls you found in the polling websites described above.  How do these results differ? How are they the same? Can you explain the differences and similarities?
    – Were the results of your poll what you expected? Why or why not?
    3.  **Assess** the polling process. As part of your assignment package, provide an analysis of the following points in approximately 225-250 words:
    – What problems did you encounter?
    – How challenging was it to craft neutral, unbiased questions?
    – Did you ask the right questions? How might you have re-worded your questions to get better results?
    – Which are better for polling: Open-ended questions, or close-ended questions? Explain your answer.
    – What are your overall thoughts about polling? Do we rely too much on polling? How might we otherwise gain information about what people prefer?
Submit your assignment as a file upload.

    ### Instructions
    #### Step 1: Review the Case Study
    **Organization:** Love Health Clinic
    – **Location:** Suburban area with a growing aging patient population
    – **Background:** Love Health Clinic is a for-profit healthcare organization located in a suburban area. Established in April 2005, the clinic has been a cornerstone of the community for many years. During this time, the demographics of the community have shifted significantly, with a notable increase in the aging population. Despite this change, the clinic has not yet adapted its services to meet the needs of this growing demographic. There are two more healthcare organizations in the area that offer specialized services that cater to the aging population, but because they are further away, they often pose transportation issues for this population.
    – **Current Services:** Love Health Clinic offers a variety of services including pediatric care, maternity services, physiotherapy, sports medicine, emergency trauma care, and orthopedics. These services are designed to address a wide range of health issues such as chronic and acute illnesses in children and adults, pregnancy and childbirth-related concerns, musculoskeletal injuries and conditions, sports-related injuries and rehabilitation, emergency medical situations and trauma, post-surgical recovery and rehabilitation, and bone and joint disorders.
    – **Financial Position:** Financially, Love Health Clinic is in a stable position. The clinic's revenue has been consistent, with a steady stream of income from its diverse range of services; however, over the last two years they have not experienced a growth in revenue or patient numbers. The clinic has not recently performed any audits to make sure they are compliant with any new healthcare regulations and policies that might affect the clinic's operations or financial stability. As economic financial challenges continue to rise, the clinic's board of directors is concerned that revenue will start to decline if the clinic does not change or modify its future direction. There is believed to be significant potential for growth by expanding services to better cater to the aging population. The organization needs to identify a new long-term sense of direction and purpose and roadmap for getting there.
    – **Strategic Direction:** Managing population health involves significant planning, strategies, and interventions. As the newly appointed healthcare administrator, your number one priority is to enhance the clinic's growth by addressing the needs of this growing demographic over the next year.
    **Current Situation:**
    – **Leadership:** The clinic is led by Dr. John Smith, the current chief operating officer (COO), who has an autocratic leadership style. This has led to a lack of innovation and growth within the clinic, as staff members feel their input is neither valued nor considered.
    – **Services:** The clinic offers a range of general healthcare services but lacks specialists who focus on geriatric care. This gap in services means that the clinic is not fully equipped to address the specific health needs of the aging population.
    – **Insurance Contracts:** Love Health Clinic does not have any standing contracts with Medicare Advantage plan partners. This limits the clinic's ability to attract and serve patients who rely on these plans for their healthcare needs.
    – **Community Perception:** The clinic is increasingly perceived as somewhat outdated and unresponsive to the changing needs of the community. This has resulted in a slowdown in patient numbers and revenue growth.
    – **Health Professionals:** The clinic employs a variety of healthcare professionals, including general practitioners, nurses, medical assistants, and administrative staff. However, there is a notable absence of geriatric specialists, which limits the clinic's ability to provide comprehensive care to older adults.
    – **Staff Contracts:** The clinic employs both full-time and contract staff members. Contract staff include specialists brought in for specific services or temporary needs. This arrangement helps manage costs but can lead to inconsistencies in patient care and staff cohesion.
    – **Community Engagement and Outreach:** Love Health Clinic has a community engagement team that organizes health fairs, workshops, and educational sessions to promote health awareness. The clinic also collaborates with local organizations to provide free health screenings and preventive care services. Despite these efforts, the clinic's outreach programs have not been sufficient to address the specific needs of the aging population.
    – **Contract Negotiation Specialist:** The clinic has recently hired a contractor to serve as the Medicare partnership manager to negotiate contracts with insurance providers. There has been a significant barrier to establishing partnerships with Medicare Advantage plans and other insurance providers.
    **Team Members:**
    – **Project Manager – Role:** Oversees the strategic plan implementation, ensuring goals are met within the timeline, and metrics are tracked.
    – **Leadership Training Coordinator – Role:** Develops and implements leadership training programs.
    – **Medicare Partnership Manager – Role:** Secures and manages partnerships with Medicare Advantage providers.
    – **Community Outreach Coordinator – Role:** Implements community outreach programs and enhances patient feedback mechanisms.
    – **Financial Strategy Manager – Role:** Develops and implements the clinic’s financial strategy.
    – **Marketing Manager – Role:** Executes the marketing strategy.
    – **Data Analyst – Role:** Provides data insights to support decision making
    – **Administrative Support – Role:** Assists with logistical and administrative tasks.
    – **IT Specialist – Role:** Manages technology tools and systems.
    – **Chief Marketing Officer (CMO) – Role:** Oversees all marketing activities and strategies to ensure alignment with the clinic's goals.
    – **Chief Operating Officer (COO) – Role:** Oversees the clinic's day-to-day operations and ensures efficient and effective service delivery.
    #### Step 2: Outline and Organize Your Strategic Improvement Plan
Use Microsoft Word or Google Docs to outline and organize your plan. Respond to the prompts in each section.
    – **Summary of Current State**
    – In 1–2 paragraphs, summarize the current situation. Provide a clear snapshot of the clinic’s current operational situation and key challenges.
    – **Mission, Vision, and Values Statement**
    – Craft a new mission, vision, and values statement for the clinic that reflects a future-oriented strategy and provides clear long-term direction and purpose.
    – **SWOT Analysis**
    – Assess the clinic’s current strengths, weaknesses, opportunities, and threats. Identify and describe three of each.
    – **Strategic Goals**
    – Define four goals that support the clinic’s mission and vision and align to the SWOT analysis.
    – The goals should be SMART (specific, measurable, achievable, relevant, and time-bound) and short-term (achievable within one year).
    – **Strategy Development**
    – Develop a specific plan to achieve the strategic goals.
    – For each goal, identify at least two actionable steps and determine the necessary human and nonhuman resources to ensure successful implementation.
    – **Marketing Strategy**
    – Recommend a marketing strategy (cost leadership, differentiation, focus, growth, or relationship) or a mix of strategies to support the clinic’s strategic plan. Explain the recommendation and how it will help the clinic achieve its goals.
    – Identify three marketing channels to utilize (e.g., traditional channels like print media, billboards, television, and radio, or digital channels such as social media, websites, email marketing, etc.). Explain why they’re appropriate and how they will be used.
    – **Implementation Plan**
    – Establish the timeline for implementing the action plan. Indicate the specific timeframe in which each action will be implemented based on quarters (Q1, Q2, Q3 and Q4).
    – **Plan for Monitoring and Evaluation**
    – Establish a metric (e.g., patient feedback, clinical outcomes, financial reports) to monitor progress for each goal. Identify the purpose of the metric and how often it will be reviewed/evaluated.
    #### Step 3: Review and Submit Your Work
Review your work carefully and check the rubric to make sure you have met all the requirements. Submit as a Microsoft Word (.doc or .docx) file.
_By tackling challenges at Love Health Clinic, you are improving services and making a significant impact on the aging population. These skills empower you to drive positive change in other healthcare organizations._

**Discussion: Clinical Interview Role-Play (CIR) Assignment Instructions**

You will use Microsoft Teams to hold a meeting with your clinical team. You will record your session as a counselor in Microsoft Teams. The minimum length is 10 minutes and the maximum length for your counselor video is 15 minutes. You will use your interviewing skills to collect clinical assessment data from a clinical team classmate (the client) who will role-play the case study (e.g., George, Clare, Gretel, Jack, etc) given to them by your instructor. Your other clinical team classmate(s) act as supervisor(s) and observe your use of clinical skills and offer verbal feedback to you at the end of your recorded session.

For this assignment do the following:

    1) Work with your assigned clinical team to determine a day and time to do the video recordings. You can coordinate this time in Microsoft Teams in your designed channel (e.g., Clinical Team 1, Clinical Team 2, Clinical Team 3, etc).

    2) Based on the determined day and time, do your recording in Microsoft Teams with your clinical team. See “Microsoft Teams Video Recording Guidance” under Resources in this discussion in Canvas. You will only record your session as a counselor, hence the 10 to 15 minutes in length.

    3) After completing your recording, post the link in the initial post. That is all you need to do for the initial post for this discussion.

    4) Next, you will reply to two classmates that were not a part of your clinical team.

    a. First, watch their “counseling” video.

    b. Take notes as you are watching their video with timestamps. For example, “Nice work of reflecting emotion at 1.40 minutes” or “You could have offered a paraphrase here to evidence you are tracking the client at 3.00”

    c. When watching their video see the table below on skills and level of expertise that can be noted in your reply. Most skills will be at “minimal” or “proficient.” At this stage of development “Advanced” should be used rarely. You do not need to post the table below. It is only a reference.

    d. Then offer a reply that is at least 150 words focused on your classmate’s clinical skills. Offer at least 2 strengths and 2 growth areas with timestamps.
**Offer two paragraphs in your reply. The first paragraph offers strengths. The second paragraph offers growth areas.**

    e. Offer at least 1 citation with your reply. Use your textbooks or other peer-reviewed sources not websites.

    f. See the rubric for more specifics on how to reply to a classmate.
