Epidemiology is defined as the analysis and study of the causes, effects, and patterns of health and disease conditions in identified populations. Epidemiology is the basis of public health, and it shapes evidence-based practice as well as policy decisions. By recognizing risk factors for all disease targets for the accomplishment of preventive health care, and it also helps with statistical analysis, study design, collection and dissemination of outcomes. Key areas of epidemiology study entail disease transmission, causation, outbreak investigation, screening, forensic epidemiology and biomonitoring among others. Epidemiology mostly depends on other scientific disciplines, for example, biology, to better understand disease statistics and processes to ensure effective data use as well as appropriate conclusions.

When working or dealing with statistics, it is crucial to identify the different types of data which include numerical data, categorical and ordinal data; data can be referred to as actual pieces of information, which one assembles through the study. Numerical data have a meaning as a measurement, for example, a person’s weight, height, blood pressure or IQ (Fletcher, Fletcher, and Fletcher, 2014). Numerical data can be discrete or continuous. Discrete data represent items, which can be counted and they take on possible values, which can be listed out. Continuous data represent measurements and their possible values cannot be counted and instead they can be described through the use of intervals on the real number line. Categorical data stands for characteristics such as person’s marital status, gender, and hometown as well as things they like.  On the other hand, ordinal data mixes categorical data and numerical data.

Performance measurement is categorized into reliability, range, validity, interpretability, and responsiveness. Variability occurs for various reasons in a particular study, for example, the study that involves assembling data from human beings for data can become unavailable for different reasons.  The information or data shared in the National Center for Health Statistics site is way much enlightening. A lot worth borrowing for practice has been said on this site (Fletcher, Fletcher, and Fletcher, 2014). The study or site covers a statistic indicating that between the year 2000 and 2014, the COPD associated death rate decreased for both women and men aged 65 to 84 years. The rate hyped for both men and women aged 45 to 64 years and most women 85 and over.

My name is XXXX XXXX. I live just outside of Des Moines, Iowa. I received my BSN in 2003 and started out as an operating room nurse. I then transferred to the emergency department in 2006 and have been there ever since. I am currently the trauma program manager for my primary hospital which is only a regular part time job. In addition to that I work PRN in another ER, do paper and on-site trauma reviews for the Iowa Department of Public Health PRN, and am one of the clinical instructors for the capstone clinical for Kaplan’s ASN program in a critical care unit. I have been thinking about going back for my MSN-FNP for a while now and finally decided it was time. I love nursing and have been encouraged by a lot of my fellow nurses that have gone back and obtained their MSN-NP. I plan on staying in the emergency setting as an NP in our fast track area or as the trauma team NP when I am finished. On the personal side I am a busy wife and mother. My husband is a farmer and I help out where I can with the field work, but do all the books and most of the grain marketing. We have 3 children, a daughter, Cadence, that turned 4 this past Wednesday, a daughter, Sydney, that just turned 2 in August, and a son, Ryan, that is almost 9 months old. I’m looking forward to getting to know all of you throughout this exciting journey.

    Pediatric Case Study

I’m sure all of us have had a patient outcome in our practice that made us think, “what could we have done differently that could possibly gain a better outcome for the next patient?” I know I have had several over the years. In this case study I would first form a clinical question that would best help me to narrow my research focus for best results. Our text outlines using the PICOT format to form clinical questions. P is the patient population, I is the intervention or issue of interest, C is the comparison intervention or group, O is the outcome, and T is the time frame (Melnyk & Fineout-Overholt, 2015). Our text goes on to formulate the PICOT question for this study. In infants who have had cardiac surgery, how often does removing pulmonary artery catheters influence cardiac tamponade within the first week of surgery? (O’Mathúna & Fineout-Overholt, 2015). With this clinical question in mind I would start my research for evidence. One study that I read found that in order to appropriately review a topic, first one must identify appropriate databases for the review(Butler, Hall, & Copnell, 2016). For my nursing research I like to use CINAHL Plus, OVID, and PubMed Central. I would also develop a search strategy. When researching a topic I have always used the same criteria or key words in all the different databases that I use. I didn’t realize until I read one study recently that this is called a search strategy. This study noted that by using a search strategy the search can by conducted in the same way each time and will allow for the search to be replicated in the future by others to yield the same results (Aromataris & Riitano, 2014). After finding journal articles and studies from this search I would begin to examine them for the evidence that I would need to address my clinical question.

Clinical inquiry will be expressed in the PICOT format (review your text on this):

    Population

    Intervention

    Comparison

    Outcome

    Time

In order to avoid going insane, it is a good idea to use your practice setting as a starting place. Think about some of the things that you do and why you do them. Is there a better way? Are your patient’s experiencing outcomes that are less than optimal? Might there be more effective interventions?

Elements of your PICOT should be narrow and specific. For example: for population, using
    MedSurg patients
    is much too general.
    Be specific.
A good example of a specific population would be
geriatric patients status post total knee replacement
    .

After you have identifed your clinical inquiry, you need to look for any evidence you can find (at the library) to support the practice or on which to base a practice change. Post your PICOT and evidence by Sunday night.

Please–no topics involving patient falls or handwashing, unless of course these are a VERYserious issue at your practice sight.

Keep in mind that your clinical inquiry will be refined and even possibly change completely during the course of this term. It does not have to be perfect this week. I will give you feedback on your Unit 1 Discussion Topic 2 in order to help get you off on the right track.

    PICOT assignment-Neuropathy Pain

For my PICOT assignment, I will research adult patient experiencing peripheral neuropathy pain and treatment with traditional pain medication vs intravenous immune globulin(IG). Many causes of neuropathy are unknown, but many condition such as diabetes, traumatic injury, infections, and toxin can trigger it. Treatment for these conditions includes pain medication, IV Immune Globulin, steroids, plasmapheresis and other medication. There are not many publish research related to these treatments. The peripheral Neuropathy Outcome Measures Standardization (PeriNomS) was conceived to select and developed a minimum core set of outcome measures to evaluate the disease course and the effects of therapeutic interventions (Bergh, 2015).

Traditional pain medication and other medication use for neuropathy treatment have not been effective.  The current medication approved by the US Food and Drug Administration(FDA) for treatment are pregabalin, duloxetine, and tapentadol. These medications have not proven to be effective and treatment discontinuation by patients are common (Yang, 2015)

I currently work in home infusion and care for patient with this condition. treatment with Immune Globulin is very effective and help with pain relief and improvement in some of the other symptoms of this condition.  The infusion is administered weekly-every 3 months in my experience. I understood from past information that the medication is most effective when administered every four weeks. This medication is very costly to all payor, and therefore getting the treatment can be difficult at times.  Currently there are not a lot of research available that document the positive effects of IG treatment over long periods.  In my experience patients report of how they are feeling after the treatment and how they are feeling when it’s time for their treatment seems to be one of the criteria for ongoing treatment. I will research this topic, gathering current information on IG use and barriers in treatment of neuropathy pain.

  • LGBT and Transgender health needs in the US essay

LGBT and Transgender health needs in the US essay

    Dianelys:

The rise of transgender people has formed the topic of most conversations in the United States and globally too. Lack of adherence to transgender rights has formed a great percentage of that conversation. The rights to access healthcare facilities ad be part of particular institutions have been denied due to their choice of lifestyle.

Medical experts and physicians need to be widely informed about LGBT and Transgender health needs because they continue to suffer in silence. These medical personnel need to be educated on the health needs of the LGBT because for a long period, individuals from this community have been discriminated and have been labelled as having mental disorders. This has formed part of the major reason why they do not enjoy such fundamental rights. (Hafeez, Zeshan, Tahir, Jahan, & Naveed, 2017 ). The long history of denying people from this community health services has driven them to harmful practices aimed at harming themselves and people close to them.  Secondly, although they do not suffer from specific diseases, they face a number of health disparities which affect them adversely. These disparities arise from legal issues, discrimination and lack of healthcare policies to accommodate members of this community.

To address these issues, there is need to create an enabling environment for all LGBT people. Support groups where they are able to frequently meet and talk about their problems while coming up with in-house solutions. One cannot stress on the importance of educating the masses about their healthcare needs. In (Martos, Wilson, and Meyer, 2017) apart from general healthcare practices, there are particular issues that face transgender community, they are referred to as transgender care: it entails special mental and physical care for such patients. Transgender patients particularly require such specialized treatment as they transition from one gender to another.

    Yuritza:

The main theme running through discussions of healthcare for the LGBT community is that there is a lack of knowledge about the types of healthcare issues that the community faces. This then results in healthcare disparity and inequities. Along with more knowledge, healthcare workers need to become more sensitive and welcoming to the LGBT community. Because of past feelings of lack of acceptance, judgment and discrimination, this cultural group tends to have a higher rate of chronic illnesses such as hepatitis B and C and a higher prevalence of cancer and cardiovascular disease than heterosexuals do. Johnson (2015) of
    Modern Healthcare
says, “Many LGBT patients say they’re reluctant to visit providers that do not offer services specific to the LGBT population. As a result, many LGBT people may delay or forgo seeking care to avoid what they fear will be an uncomfortable experience with clinical staff” (Johnson, 2015). This attitude on the part of healthcare providers must change, and advanced practice nurses can lead the way.

Advanced practice nurses can specifically address the issue of health disparities among LGBT people through research. Cahill and Makadon (2014) of
    LGBT Health
(2015) cite an Institute of Medicine (IOM) Report from 2011 that found health disparities “such as the prevalence of sexually transmitted infections and HIV, and the high rates of behavioral health issues, including suicidal ideation and attempts, often related to stigma, discrimination, bullying, and hate crimes” (p. 1). Other findings from the report include the fact that lesbians are more likely to be overweight or obese than heterosexual and bisexual women (Cahill & Makadon, 2014, p. 1). Other issues such as doing prostate gland exams on trans women and breast exams on trans men may not occur to healthcare providers, but these sorts of issues can be addressed through research, part of an evidence-based practice, something advanced practice nurses should be aspiring too.

Learning about the LGBT community and doing research on ways to lower the number of health disparities comply with many of the essentials for a Master’s degree in Nursing Science. Doing research on LGBT patients provides a “background for practice from science and humanities, demonstrates organizational and systems leadership, translates into quality improvement and safety, and translates and integrates scholarship into practice. Doing research on how to best help the LGBT community provides a background for research and integrates findings from that research into nursing care across diverse settings. It will also work to change the culture that has thus far created a reluctance among the LGBT community to seek healthcare. This type of research is ethical and demonstrates care because searching for ways to help is seeking to eliminate disparities. By finding solutions to the healthcare problems that the LGBT community faces, advanced practice nurses use what they have learned from research and study to change practice norms for healthcare providers who work with the LGBT community.
